What does Matthew 15:7 mean?

Jesus is calling out the religious leaders who have just criticized his disciples for eating without washing their hands according to tradition. Rather than arguing about the rule itself, Jesus goes straight to their real problem: they care deeply about outward appearances and human traditions while ignoring God's actual heart. He's about to quote Isaiah to show that the ancient prophet saw this exact pattern of hypocrisy coming, so this isn't a new problem or a surprise to God. It's a sharp wake-up call, but it comes with an implication of mercy too—if Isaiah warned about it long ago, there's still time to change course.
