What does Matthew 22:39 mean?

Jesus ties this commandment directly to the first and greatest one, about loving God with all your heart. He's saying that loving others well isn't separate from faith, it's inseparable from it. The phrase "as yourself" doesn't mean you should neglect your own wellbeing, but rather that the care and kindness you naturally want for yourself should flow toward others too. In hard seasons, this can feel impossible, but it's meant to be a north star for how we treat people, not a guilt trip when we're struggling.
